About This Learning Program

Welcome to the Connecting and Coordinating Person-Centered Care learning program! This online program was developed as continuing education learning modules for nurses. The main goal of the program is to help nurses develop an understanding of care coordination in the current health care delivery system. This will ultimately improve nursing practice across the health care continuum, while improving population health and patient-centered care.

Once you have gained access to the program (following the pre-course survey), you will see 4 mini courses on the edge.edx.org/dashboard. The 4 mini courses and their awarded CEUs for completion are:

  • Care Coordination: General Content (6.75 CEUs)
  • Care Coordination: Acute and Trauma Care, and Behavioral Health (4.75 CEUs)
  • Care Coordination: Population Health (3.75 CEUs)
  • Care Coordination: Outcome Measures, Data, and Analytics (4.25 CEUs)
  • You can begin exploring the program by clicking on any of the 4 mini courses. The program overview of each mini course will provide a brief overview of that course.

    At the end of each mini course, you will find a “Self-Assessment,” which will help assess your understanding of the content. The self-assessments are not graded but you must complete them in order to generate the course Completion Certificate and to be able to take the CE Eval and Post-Course Survey.

    To Earn Continuing Education Units (CEUs)

    Nurses may receive up-to 19.5 CEUs for this learning program if they complete all 4 mini courses. Nurses may pick and choose which mini course(s) to take, so the actual number of CEUs awarded will be dependent on which mini course(s) is/are completed.

    To obtain the CEUs, the link to the “CE Eval and Post-Course Survey” will become available on the Completion Certificate form page once the content sections and self-assessment have been completed for that mini course. Learners will need to follow that link and take the survey.

    Following submission of the CE Eval and Post Course Survey, learners will receive an email from UMSON’s Continuing Education Department with their CEUs within a few weeks. If you have any questions about this, contact us at 410-706-2283 or nrsccic@umaryland.edu.
